This document contains the fault study occurred at the Ex Estación Maule, belonging to the Empresas de Ferrocarriles del Estado, which is located in the VII Region of Chile, where the details of the fault are explained. Also a photographic is exposed of the damage caused by the incident. Due to the failure of the ceramic insulating structure located in the PK260-5 rail gantry, an improper circulation of current from the catenary supply system to the triangular supports of the gantry was initiated and later it was diverted in the direction of the facilities of the Technical Premises before mentioned. The aforementioned, added to the fact that the ground protection system was not in operation due to the theft by third parties of the copper bars belonging to the protection, generated a fire in the facilities of the Technical Site, interruptions in the operation of the system, losses of equipment, devices and decrease in the quality of service maintained by the state company. Detailed technical used by the company, such as: voltage level, current and contactor systems. Characteristics present at the time of failure to expose the operating conditions of the system. The main and secondary causes that led to the occurrence of the electrical fault are identified. The operating conditions and infrastructure present in the fault zone are determined. The ceramic insulating elements are defined and analyzed, devices that directly influenced the occurrence of electrical failure, explaining characteristics, properties and forms of degradation suffered by these types of insulators. A chronology of events and actions carried out that took place from the beginning of the failure in the PK260-5 portico to the moment in which the power supply cut by EFE officials was carried out. The DigSilent PowerFactory software was used to carry out a simulation that would demonstrate the magnitude of the current generated by the incident. With the previous results it was possible to determine in an estimated way the temperature value reached in the fault zone. The concept of reliability is detailed with its respective indexes and parameters, whose data can be implemented to maintenance plans aimed at increasing the useful life of elements and devices. In addition, a series of recommendations are presented to prevent the failure from happening again, increasing the reliability and security of the system
